Care Coordination refers to the management of a patient's care during the transition from one care setting to another. For our patients, that means a focus on your care when you transfer from the hospital to Wexner Rehabilitation, and from Wexner Rehabilitation to home.
Before you arrive, our Care Coordination Team coordinates every detail of your admission to Wexner Rehabilitation. Our admissions office communicates with your hospital discharge planner, reviews your clinical records and verifies your insurance benefits.
While you are here, your healthcare team - including doctors, nurses, therapists, social workers and dieticians - will work with you and your family to develop personalized goals and strategies to get you back home as close to your prior level of function as possible. At the same time, our case managers will facilitate communication between you, your healthcare team and your insurance company to assure your medical needs are met and you receive your maximum insurance benefit.
We know that patients who are more involved in
their health care are happier with their health
care decisions and are more likely to follow
treatment plans, which can lead to
better health outcomes.
When you are ready to go home, our social workers will coordinate your discharge to make sure you have the education, equipment and services you need for a smooth transition and continued success. They can connect you with Wexner Home Care
and, if appropriate, arrange for in-home therapy assessments prior to your discharge.
